Credentialing Service

Credentialing is the process of verifying a provider’s qualifications to ensure that they can provide care to patients. Most health insurance companies require this process including CMS/Medicare, Medicaid, and Commercial plans, as well as hospitals and surgery centers.

The providers credentialing process is completed by verifying all of a provider’s documents to ensure that they are valid and current. These include their medical license, malpractice insurance, and DEA.

Dr. Rivera is a podiatric surgeon who specializes in surgeries of the foot and ankle. His practice concentrates on problems specific to the foot and ankle, including fractures, bunions, hammer toes, degenerative and post-traumatic arthritis, rheumatoid arthritis, neurological disorders, and sports-related injuries.

Dr. Rivera was born and raised in southern California. He earned his medical degree from Kent State and completed his podiatric surgical residency at Liverpool City Hospital. During his residency, he was involved in numerous research projects and presentations regarding the foot and ankle.

He currently resides in Grass Valley with his wife, Kelly, and their three children, Madden, Audrey, and Conan. He enjoys spending time with his family, golfing, and practicing jiu-jitsu.

Dr. Payne is a physician who enjoys working with patients to achieve their optimal health and keep them moving. He treats various aspects of foot and ankle pathology, both conservatively and surgically.

Dr. Payne was born and raised just north of Salt Lake City, Utah. He grew up playing sports including soccer, baseball, and his favorite—basketball. His interests include spending time with his wife (Kelly) and four children (Findlay, Elsie, Clancey, and Stockton). He also loves traveling, movies, golfing, and playing basketball whenever he gets a chance. Dr. Payne is a proud Eagle Scout.

Dr. Payne attended Kent State University – College of Podiatric Medicine and completed his surgical residency training in the Henry Ford Health System in the Detroit, Michigan area.

Trevor Haynes, DPM, FACFAS, FACFAOM

Trevor Haynes, DPM, FACFAS, FACFAOM, brings a wealth of experience and knowledge to Elite Foot & Ankle Associates, which he happily shares with his patients in Sandy and Portland, Oregon, allowing them to move better through life. Whether it’s a smaller problem, like an ingrown toenail, bunion or foot fracture, Dr. Haynes provides every patient with quality care knowing that the ability to move without pain is paramount when it comes to quality of life.

Originally from Idaho, Dr. Haynes attended Weber State University in Ogden, Utah, graduating with a Bachelor of Science Degree in Microbiology. From there, he attended podiatry school at the Ohio College of Podiatric Medicine (now the Kent State University College of Podiatric Medicine) in Independence, Ohio.

After earning his medical degree in 2012, Dr. Haynes moved his family to Houston where he completed a three-year foot and ankle reconstructive surgery residency at West Houston Medical Center. While there, he trained under the renowned Dr. Samuel Mendicino, as well as 30 other podiatrists and orthopedists.

In 2015, Dr. Haynes completed his prestigious residency and moved his family to Brookings, South Dakota, where he joined the Avera Medical Group. Dr. Haynes performed various bunion, hammertoe, and other foot and ankle surgeries. Dr. Haynes enjoys treating sports injuries and correcting bunion deformities. He also has extensive experience in wound care of the lower extremity.

After nearly three years in South Dakota, Dr. Haynes felt it was time to move his family from the Midwest back to the area he grew to love, the Pacific Northwest.

Dr. Haynes is board certified in foot surgery by the American Board of Foot and Ankle Surgery and is board certified by the American Board of Podiatric Medicine. He is a fellow of American College of Foot and Ankle Surgeons and a Fellow of the American College of Podiatric Medicine. He is also a member of the American Academy of Podiatric Sports Medicine, American Podiatric Medical Association, Oregon Podiatric Medical Association, and American Association of Podiatric Sports Medicine.

Dr. Haynes performs surgery at Adventist Health Portland, Providence Willamette Falls, Legacy Mt. Hood and Mt. Scott Surgery Center.

Dr. Haynes has been married for 20 years to his beautiful wife Karie, and they have four children: Jaden, KynnLee, Grayson, and Rhettick. Dr. Haynes enjoys sports, the outdoors, and spending time with his wife and children.
